Masks must be worn properly and at all times.

If you do not have one – we will be happy to provide you with one.

If you refuse to wear a mask … you will be refused entry and/or asked to leave.

COVID testing is now available for our patients and the public

Starting August 9th. (Mon to Fri – 8am to 4pm) Just pull into our parking lot, look for the numbered parking spots and representatives of Bluewater Diagnostics will test you while you are in your car. There is no out of pocket cost for you to receive a test. If you have insurance it will be billed, or if you are not insured the cost is covered by federal programs. You may call for pre-registration before you arrive in order to save time while you are here.

If you feel sick with symptoms such as fever, cough, or shortness of breath, we ask that you call first!

Fairview Community Health Center is committed to serving our patients throughout this pandemic by following CDC and DOH recommendations and guidelines.

If you are told to come into the clinic, you may be screened for any signs of a fever, other risk factors, and asked about your symptoms (if any).

All patients may be REQUIRED to wear a protective mask while in the clinic building. We will provide a mask for you to borrow if you do not have your own.

Only the patient into the building whenever possible. With the exception of a Caregiver and child.
