Fairview CHC's COVID-19 Rules

Fairview Community Health Center is committed to serving our patients throughout this pandemic by following CDC and DOH recommendations and guidelines.

If you are told to come into the clinic, you will be stopped at the main entrance and screened for any signs of a fever, other risk factors, and asked about your symptoms (if any).

All patients may be REQUIRED to wear a protective mask while in the clinic building. We will provide a mask for you to borrow if you do not have your own.

Only the patient into the building whenever possible. With the exception of a Caregiver and child.


  • In partnership with Bluewater Diagnostics to offer COVID-19 Testing. They are located on the outside westside of the building Monday to Friday 9 am to 4pm. results are available usually in about 24 to 36 hours.
