Masks are still REQUIRED to enter our facility

Masks must be worn properly and at all times.

If you do not have one – we will be happy to provide you with one.

If you refuse to wear a mask … you will be refused entry and/or asked to leave.

Especially now with the new Delta Variant being so much easier to transmit …

If you feel sick with symptoms such as fever, cough, or shortness of breath, we ask that you call first!

As of 3/12/2021 – We are NO longer offering parking lot COVID testing to the general public. We are now only able to accommodate our current and existing patients with COVID testing services during their normal provider visits.

 

Fairview Community Health Center is committed to serving our patients throughout this pandemic by following CDC and DOH recommendations and guidelines.

If you are told to come into the clinic, you may be screened for any signs of a fever, other risk factors, and asked about your symptoms (if any).

All patients may be REQUIRED to wear a protective mask while in the clinic building. We will provide a mask for you to borrow if you do not have your own.

Only the patient into the building whenever possible. With the exception of a Caregiver and child.
