Our partner, Bluewater Diagnostics , we be discontinuing  drive up COVID testing at our location on Friday – 3/11/22


 

Masks are RECOMMENDED to enter our facility

Mask are still REQUIRED if you are seen in our Triage Department

If you choose to wear a mask, it should always be worn properly covering the nose & mouth.

If you do not have a mask and would like one , we will be happy to provide you with one.

Fairview Community Health Center is committed to serving our patients throughout this pandemic by following CDC and DOH recommendations and guidelines.

If you feel sick with symptoms such as fever, cough, or shortness of breath, we ask that you call first!

During normal business hours (M-F 8 am to 5pm) –

If you are told to come into the clinic, you may be screened for any signs of a fever, other risk factors, and asked about your symptoms (if any).

Any or all patients MAY BE REQUIRED to wear a protective mask while in the clinic building if the medical staff feels you may be contagious or are being seen in our TRIAGE Department.

We will provide a mask for you to borrow if you do not have your own.
