Here at Fairview Community Health Center we are committed to providing high quality healthcare to all patients in our community even if you are uninsured or under-insured – regardless of your income level. Our sliding fee discount application is simple and easy. You will be given a 2 page application upon check in. To complete the form you will need is proof of your household size and total income. This can be in the form of paystubs, tax returns or a written statement from an employer or personal reference.
- A minimum $25 nominal charge is required at registration from all self-pay patients.
- All lab services sent to an outside laboratory will need to be paid for at the time service is delivered, unless other payment arrangements have been made.
