What is a Federally Qualified Health Center?

“Federally Qualified Health Centers are community-based health care providers that receive funds from the HRSA Health Center Program to provide primary care services in underserved areas. They must meet a stringent set of requirements, including providing care on a sliding fee scale based on ability to pay and operating under a governing board that includes patients.” – Health Resources & Services Administration

Again Fairview CHC has received HRSA quality awards for the care they have provided in the past year as well as FSHCAA deemed status.

Fairview Community Health

A Medical Home

Fairview Community Health Center exists to provide affordable access and quality medical healthcare for anyone in the greater Bowling Green / Warren County area of Southern Kentucky. We are a medical home where a patient develops a relationship with a health care provider that will provide excellent access to affordable healthcare. It is well documented that a healthcare relationship with a healthcare provider over time leads to better outcomes and improved health.

Safety Net Provider

Fairview Community Health Center is the safety net provider for the Bowling Green / Warren County area (and beyond). We currently serve over 15,000 patients and in the 2023 fiscal year, we provided care via 43,347 patient encounters. We take most all major insurance plans as well as Medicare and Medicaid. While we are NOT a FREE clinic; we do provide a sliding scale fee structure for all self-pay patients.

Onsite Translation

 We offer onsite translation for Spanish, Bosnian, Burmese and some dialects such as Zho-mi, Chen & Khareni), as well as effective communication for those with disabilities. Almost any language is available to us through real-time telephone translation via our language line partners.

Who are We?

Fairview Health Center is a 501c3 non-profit, federally qualified health center that has served the local community since 2001. Fairview Health Center strives to adhere to the highest quality standards of patient care, regardless of patients’ insurance status or ability to pay. Employees of Fairview Health Center are deemed to be federal employees qualified for protection under the Federal Tort Claims Act (FTCA).

Accessible

Fairview Community Health Center strives every day to provide access to excellent quality healthcare. Whether it is a language barrier, a physical barrier or a financial one, Fairview CHC works to improve a person’s access to a better life. We advocate for affordable healthcare with elected officials at the state and federal level to maintain and improve access whenever we can by teleconference, correspondence and trips in person to Frankfort and Washington DC as well as our membership in the Kentucky Primary Care Association.

Affordable

Fairview Community Health Center is not a Free Clinic, however we are designated as a safety net provider in the area by the Federal Government. This gives us access to funding & the flexibility to work with our patients, both insured and un-insured, to find a financial solution that does not result into a financial burden for them. All fees for self-pay patients can be adjusted based on the patient’s ability to pay and their income based as compared to the federal poverty sliding scale guidelines. We will not refuse services to those who are unable to pay. However, we do ask for a $25 nominal charge to all that can afford it in order to contribute to the mission of Fairview CHC.

Accountable

Fairview Community Health Center is governed by a Board of Directors of which 50% of the members must be consumers/patients of the Health Center. Annually we report to the Health Resource Service Administration ( HRSA) on our progress achieving quality measures and improved health outcomes for our patients.

Fairview Community Health Center is staffed by dedicated and caring people who believe passionately about equal and easy access to quality healthcare for all. Our Doctors and Nurse Practitioners do their very best to make Fairview CHC a medical home for all who need care.

The Medical Center / Women’s Health Specialist

Women’s Health Specialist provide all of our prenatal and OB/GYN care for soon to be and new moms.

Norton’s Children’s Hospital

Norton’s provide prenatal ultrasound services they also take referrals for any of our high risk and/or complicated pregnancy patients and new babies.

WKU Dental Clinic

Periodically we offer dental services to our patients through the WKU mobile dental van.

Matthew 25

Fairview CHC is happy to partner with Matthew 25 AIDS Services. supports, educates, and treats those infected with and affected by HIV/AIDS.

Barren River District Health Department

Barren River District Health Department is a key partner to us and a vital player in health and safety in and around our region
